We Clean Everything, (“the Company”) may obtain information about you from a consumer reporting agency for employment purposes. This information may be obtained in the form of a consumer report and/or an investigative consumer report.
A consumer report may include information regarding your:
- Criminal history.
- Credit history (if applicable).
- Driving record.
- Employment history.
- Education.
An investigative consumer report may include information obtained through personal interviews regarding your:
- Character.
- General reputation.
- Personal characteristics.
- Mode of living.
These reports may be obtained at any time after receipt of your authorization and, if you are hired, throughout your employment where permitted by law.
You have the right to request additional disclosures regarding the nature and scope of any investigative consumer report by submitting a written request to the Company.
You also have the right to request a copy of your report and a summary of your rights under the Fair Credit Reporting Act.
By signing below, you acknowledge that you have received this Disclosure.

I understand that these reports may include information regarding my:

- Employment history.
- Education.
- Other background information permitted by law.
I authorize all individuals, organizations, and agencies to release such information to the Company and/or its designated consumer reporting agency.
I understand that:
- This authorization will remain valid throughout my employment.
- I have the right to request information about the nature and scope of any investigative report.
- I may request a copy of any report obtained.
I release the Company and all parties involved from any liability arising from the procurement of such reports, as permitted by law.
